ABSTRACT :
Against the background of personalized medicine, understanding a patient’s ability to withstand stress, defined as their biological reserve, is crucial for determining treatment strategies and prognosis. Traditional metrics such as the Karnofsky Performance Status and the ECOG Performance Status, while useful, often rely on subjective observations and may fail to capture the complex interplay between different organ systems. This case report illustrates the practical application of the Biological Reserve Diagram, an integrative visual tool designed to quantify the structural and functional resources of the human body across twelve distinct physiological domains.
We present the evaluation of a patient using the Biological Reserve Diagram, which employs a 0–10 scoring system, where a score of 10 represents maximal physiological integrity. The assessment revealed a patient with a predominantly high level of resilience. The subject demonstrated optimal functional capacity in key areas, achieving a perfect score of 10.0 for the Central and Peripheral Nervous System and 9.86 for the Cardiac System. These values placed the patient predominantly within the Dark Green Zone, indicating robust adaptive capacity and preserved structural integrity.
Despite the favorable overall profile, the Biological Reserve Diagram identified subtle, subclinical deficits that might otherwise have remained undetected. Specifically, visible indentations were observed within the Light Green/Yellow Zone (scores between 5.0 and 7.9) for Specific Biomarkers (7.43) and Blood Constants (7.86). These lower scores may indicate latent inflammatory processes, early metabolic disturbances, or nutritional deficiencies. Although the patient did not reach the critical Red Zone, these findings suggest that the biological buffer may already be under strain, warranting close monitoring to prevent progression toward frailty.
This case highlights the value of the Biological Reserve Diagram in identifying the discrepancy between apparent clinical health and actual physiological reserves. By providing a dynamic, multiorgan visualization of biological resilience rather than a single performance score, this approach enables clinicians to detect otherwise silent vulnerabilities at an early stage. Such information may facilitate targeted interventions aimed at restoring biological reserve before the development of overt organ dysfunction.
